Child-rearing Support Special Allowance Application Procedure

This is to notify all persons concerned of the application procedure for receiving the Child-rearing Support Special Allowance. It is required that the head of the household file the application for this benefit during the prescribed period.
Eligibility
As of base date, February 1, 2009, head of the families with eligible children who falls under 1 or 2:
1. Those who are registered on the Basic Resident Register of Okaya City.
2. Registered foreign residents of Okaya City. (Illegal immigrants and short-term residents are not eligible.)
Children who are eligible for the benefit
Second child (and succeeding ones) born between April 2, 2002 and April 1, 2005.
Note: Second child is defined as the succeeding child after the first one who was born not earlier than April 2, 1990.
Amount to be received
36,000 yen per eligible child
Sending of Application Forms
The city will send the application form to all eligible beneficiaries. The scheduled date of sending by post is March 16, 2009. (Delivery may be delayed for certain reasons)
Note: Heads of the household living separately from the eligible children and the first child (i.e. the children are not registered in Okaya City) will not receive the application form. Please inform the city hall if this is your situation. If this is the case, the condition is for the eligible child to be supported by the same person.
Application Procedure
You can either apply by post (using the enclosed return envelope) or directly submit your application to the city hall.
You are required to attach copies of documents that can prove your identity (driver?s license, resident card, passport, alien registration certificate) and a copy of your bank account passbook (showing your name and account details) when filing your application.
Please read the directions attached with the application forms for details about filling out the forms, and ways to submit your application.
Application Period
Six-month period starting from March 18, 2009 (Wednesday) to September 18, 2009 (Friday)
Endowment Period
The city is still in the process of arranging the payments to be made by the end of March 2009. After submitting the forms, your application will be examined. You will then receive a separate notice regarding the scheduled date of deposit to your bank account.
