Child Allowance Approval Application (Present Status Notification)
Those who are currently receiving child allowance are required to submit the Child Allowance Approval Application (Present Status Notification) "Jidou Teate Nintei Seikyuusho (Genkyou Todoke)" once a year. This is to state the current status as of June 1st every year, and to confirm if the applicants are eligible to remain receiving the child allowance.
Those people covered by the system have been notified in early June thus please make sure to submit the application before the deadline.
Please keep in mind that the payment will be suspended or discontinued if you fail the submission.

- Photocopy of health insurance card : Contributors to Employee Pension Plans
- Proof of Income "Shotoku Shoumeisho" : Those who moved into Okaya City on and after January 20th, 2008. (Proof of Income and Support "Shotoku oyobi Fuyou Shoumeisho" of 2007 at previous address.)
